1. I, <full name>, <son/daughter/wife> of <relative’s name>, aged <current age> years, born on <DD-MMM-YYYY>, of religion <religion>, residing at <address>, do hereby revoke all prior Wills and declare this to be my LAST WILL AND TESTAMENT.
2. I am of sound mind and make this Will of my own free will, without pressure or influence.
